Our Kirklees Well Programmes
Addressing health inequalities is our top priority. It can be complex, but progress can be made through strong partnerships, concerted focussed effort and a joined-up approach between organisations. Our Well Programmes cover the whole ‘life course approach’ and are the place where we come together to work on focussed initiatives, join up services and plan together for the best outcomes for people.

We plan to take a ‘life course’ approach, reflecting on how people’s health and care needs change over their lifetime starting well, living well and ageing well. We also have a programme for dying well at any point in a person’s lifetime.
Improvements in mental wellbeing, reducing inequalities and the personalisation of services and care are threads which will run through this work.

- Starting well focuses on Families Together and the Whole Family Early Support Offer through Family Hubs and Start for life.
- Living Well focuses on prevention and long-term conditions.
- Ageing Well focuses on proactive care, frailty and creating age friendly communities.
- Dying Well focuses on bereavement support, advanced planning and care at home.
