The Kirklees Health and Care Partnership is new, but our partner organisations have worked together for many years to design, plan and deliver services, and improve health and care for local people.  Read about some of our initiatives in the case studies below.

Urgent Community Response (UCR) Service

This service – focusing on the elderly, vulnerable, frail and those with complex needs – aims to keep people at home longer, and reduce hospital admissions.

Enhanced support in care homes

GPs, working together with care homes and community services, now deliver enhanced health in care homes. This ensures consistency, improves the quality of care, increases the support provided to care home staff, and focuses on preventing ill health.

Discharge to assess

When people no longer need hospital care, they should be supported to return home, where any health and care requirements can be assessed rapidly and dealt with in the community. By working together across Kirklees we can ensure people get the right care, in the right place, at the right time.

Working to enhance end-of-life care in care homes

Kirklees Council and The Kirkwood have developed a training programme to improve the quality of care in care homes.

COVID-19 vaccination programme

We worked with partners across the health and care system, local communities and third sector to rapidly roll-out COVID-19 vaccinations.

COVID-19 oximetry at home

We designed this service to provide remote monitoring for high-risk patients with COVID-19 symptoms. We loaned eligible patients a pulse oximeter to monitor their oxygen levels at home and report results via a dedicated phone line or smartphone app.

Mental health trailblazers

Kirklees received funding as a ‘trailblazer’ to set up four Mental Health Support Teams to work across more than 60 educational establishments. The teams comprise specially-trained expert practitioners supporting children and young people with mild to moderate mental health issues.
