When people no longer need hospital care, they should be supported to return home, where any health and care requirements can be assessed rapidly and dealt with in the community. By working together across Kirklees we can ensure people get the right care, in the right place, at the right time.

Keeping patients in hospital longer than necessary can lead to worse outcomes, particularly for older people. It also impacts on the wider health system by increasing the time others have to wait for treatment.
