This plan sets out the priorities for the Kirklees Health and Care Partnership over the next five years. It focuses on those areas where strong partnership working is needed to improve health and care outcomes and reduce inequalities.
We understand that poor health outcomes and inequalities are often the result of factors outside the control of the NHS and social care services. Housing quality, poverty, employment, education, physical environment, and ethnic background can all impact on an individual’s health and wellbeing.
Addressing health inequalities and outcomes will be difficult. But we believe progress can be made through a strong partnership approach and focusing on areas where we can make a real difference.
This plan supports the Kirklees Health and Wellbeing Strategy, West Yorkshire Integrated Care Strategy and Joint Forward Plan.
Over the longer term, we plan to take a ‘life course’ approach, reflecting on how people’s health and care needs change over their lifetime (Starting Well, Living Well, Ageing Well and Dying Well). Improvements in mental wellbeing is a thread which will run through this work.
Our commitment is to ensure we continue to focus on the right things means that the plan will be reviewed annually.

How we are strengthening our partnership:
- We have set up a delivery collaborative* and a partnership forum** to provide mechanisms for problem solving and innovation.
- We have formal alliances between organisations working in the areas of mental health and end of life care.
- We will continue to explore opportunities for organisations to work together, share resource, and develop combined plans.
- We have a voluntary and community sector partner – TSL Kirklees – which enables organisations across Kirklees to work strategically.
- We have a Kirklees Workforce Development Strategy which is being delivered by a Workforce Steering Group jointly with Calderdale.
- We have a Kirklees Digital Health and Care Board.
* The Kirklees Health and Care Partnership delivery collaborative brings together health and care service providers to develop a strategic vision and enable support through collaboration. Members include NHS hospital trusts and ICB, ambulance service, GPs, Kirklees Council, Locala Health and Wellbeing, Local Care Direct, The Kirkwood and TSL Kirklees.
** The Kirklees Health and Care Partnership Forum brings together a wide range of partners from across the borough to inform and support the work of the Kirklees ICB Committee and Health and Wellbeing Board. It will help to ensure that a broad range of organisations are actively involved in the process of decision making in Kirklees.
