We are a partnership of health and care organisations that come together to plan and deliver joined-up services and to improve the health and wellbeing of people who live in Kirklees. Our focus is on preventing ill health, improving health and healthcare, reducing health inequalities, making the best of our collective resources, and putting communities at the heart of all we do.
Our partnership brings together the local NHS, other health and care providers, Kirklees Council, Healthwatch, and community and voluntary sector organisations, to arrange and deliver services for the 440,00 people who live in Kirklees. Find a full list of organisations who are part of the partnership on the Our partners page.
We support the delivery of the Kirklees Health and Care plan and are committed to achieving the following four aims:
- improve outcomes in population health and healthcare
- tackle inequalities in outcomes, experience and access
- enhance productivity and value for money
- help the NHS support broader social and economic development
Carol McKenna, place-based lead for Kirklees Health and Care Partnership, explains more about the partnership and its work.
Kirklees Vision: No matter where they live, people in Kirklees live their lives confidently and responsibly, in better health, for longer and experience less inequality.
Integrated care systems
Integrated care is about giving people the support they need, joined up across local councils, the NHS, and other partners including care providers, the voluntary, community, social enterprise sector and charities. It involves partnerships of organisations coming together to plan and deliver seamless health and care services to improve the lives of people in their area.
Integrated care systems are geographically based partnerships bringing together organisations that meet health and care needs, improve people’s health, and reduce inequalities. There are forty-two in England – which includes West Yorkshire Integrated Care System. In West Yorkshire our integrated care system is known as the West Yorkshire Health and Care Partnership.
We are proud to be part of West Yorkshire Health and Care Partnership. We are one of five place-based partnerships in West Yorkshire alongside Wakefield, Bradford District and Craven, Calderdale and Leeds.
