Urgent Community Response Service
This service – focusing on the elderly, vulnerable, frail and those with complex needs – aims to keep people at home longer, and reduce hospital admissions.
A telephone line connects with professionals who can deal with both health and social care needs within 2 hours, or provide a tailored reablement care package within 48 hours. The service also helps people returning from hospital – ensuring they receive the right home and community support.
To date, the service has delivered over 1,000 face-to-face home appointments – with 92% of issues resolved within 2 hours.
The initiative – developed by an alliance of commissioners and providers across Kirklees – has brought partners together in a new way. As a pilot scheme, with NHS England funding, we’ve been able to test and improve the model over time.
GPs, the ambulance service and NHS 111 can now all refer into the service through a one-stop-shop approach; and we’re also developing a public self-referral pathway.
