The Well Programmes

Our Kirklees Well Programmes

Addressing health inequalities is our top priority. It can be complex, but progress can be made through strong partnerships, concerted focussed effort and a joined-up approach between organisations. Our Well Programmes cover the whole ‘life course approach’ and are the place where we come together to work on focussed initiatives, join up services and plan together for the best outcomes for people.

Three circles with arrows beginning with Starting Well, then Living Well, then Ageing Well. Thiey are surrounded by Mental Wellbeing, Personalisation, Tackling inequalities and Dying Well.

We plan to take a ‘life course’ approach, reflecting on how people’s health and care needs change over their lifetime starting well, living well and ageing well. We also have a programme for dying well at any point in a person’s lifetime.

Improvements in mental wellbeing, reducing inequalities and the personalisation of services and care are threads which will run through this work.

Four columns. The first is Starting well - Focus on families Together an d Whole Family Early Support Offer through Family Hubs and Start for life. Living well - Focus on prevention and long-term conditions. Ageing Well - focus on proactive care, frailty and creating age friendly communities. Dying Well - focus on bereavement support, advanced planning and care at home. Underneath is a bar that says Mental wellbeing and impact of bereavement considered across the life course.
