When sending out letters please consider keeping the text in similar style to other text in the document (Arial) and use size font of 14 or above. Keep sentences short and use plain English.

These documents are in easy read but may not meet your access needs. If you need them in another format like braille or larger print or find them difficult to use please let us know and we will do our best to help.

Get checked out checklist

This is to be sent out prior to the appointment so that the patient has time to complete it. It is designed to help cover relevant information to support the appointment. The checklist also contains website links for easy read resources for various topics that are mentioned in a health check. If a patient does not have internet access, they can be printed out and sent  as required.

Extra sheets for notes. Please offer to send out extra blank sheets if needed.

This should be completed by professionals following the annual health check appointment.
